Under 18's parental consent form

All self-employed lash artists or those who work in a salon should always have new client’s sign a consent. A consent form must always be read and signed prior to the service and never after! Girls under 18 always want their lashes doing. In the case of this I always get the parent to fill in the form for the minor. This way I am covering myself, I can't get a mother messaging me telling me I've done her 15 year olds lashes without her knowing.  
Signing a consent form offers protection for the client and your business.
A consent form coverers several areas which clearly outline everything the client needs to be aware of and consent to prior to the service.
Please note: Not all insurances will cover under 18's please check with your insurance.
